(“Contractor”) desires to participate in the HERO Program’s Direct Pay Program (“HERO Direct Pay”) with one or more participating distributors (“Distributors”), and, in exchange for the opportunity to be considered for participation in HERO Direct Pay, and other good and valuable consideration, hereby agrees to the terms of this Direct Pay Eligibility Agreement (the “Agreement”).
