This Amendment to the Commonwealth of Virginia Data Trust Agreement (this “Amendment”) is entered into effective as of , 20 , by and between the undersigned (hereinafter referred to individually as "Data Trust Member"), and the Chief Data Officer for the Commonwealth of Virginia ("Trustee"), on behalf of the Virginia Commonwealth Data Trust ("Data Trust"). Data Trust Member and the Trustee may each individually be referred to herein as a "Party" and collectively as the "Parties."
