This Amendment (“Amendment”), effective as of June 26, 2020, to the Medical Supplies Contract, No. 42000000-18-ACS (“Contract”), is made by and between the State of Florida, Department of Management Services (“Department”), and Henry Schein Medical, a division of Henry Schein, Inc. (“Contractor”), collectively referred to herein as the “Parties.” All capitalized terms used herein have the meanings assigned to them in the Contract unless otherwise defined herein.
