This Home Care Services Agreement ("Agreement") sets forth the terms and conditions under which LifeWorx ("Agency") will provide services to ("Client"). By Client's or Client's Representative's signature at the bottom of this agreement and/or receipt of services, whichever is first, Client agrees to the terms and conditions in this Agreement.
