Read through Clinical Guidelines paying close attention to the retention requirements and complete attached documents.□ Commander’s Authorization Memo (must be signed by the battalion commander. If commander is unavailable, the first officer in the chain of command on G-series orders is authorized to sign.)□ Managed Care Agreement□ Patient Information Sheet (DA Form 4700)□ MEDCOM Form 756--granting LRMC WRESP permission to email patients with appointmentinformation. All correspondence must be done via government email addresses. 2.
