The undersigned (hereinafter referred to as the “Subscriber”) hereby irrevocably subscribes for and agrees to purchase the number of limited partnership units of the Partnership (the “Limited Partnership Units”) for the aggregate subscription price set forth below, representing a subscription price of $1,000 (Canadian) per Limited Partnership Unit, upon and subject to the terms and conditions set forth in “Terms and Conditions of Subscription for Limited Partnership Units of Pulis Wealth Management LP 1” attached hereto (the “Terms and Conditions”) and on the terms and conditions set forth in the limited partnership agreement with respect to the Partnership made as of June 17, 2011, as amended, restated or supplemented from time to time (the “Limited Partnership Agreement”) and Offering Memorandum dated May 15, 2012 (the “Offering Memorandum”). This page plus the Terms and Conditions and the Exhibits attached thereto, are collectively referred to as the “Subscription Agreement”.
