I, on behalf of myself, my Child, and our personal representatives, heirs, executors, next of kin, administrators, successors, and assigns (collectively, “Releasors”), as the parent or legal guardian of the minor child named below (“Child”), do hereby give my full consent and approval for my Child to (a) practice or play as a member of any team that is part of or associated with Wooster Summer Fastpitch, Inc.
