Instructions for completion: This Business Associate Agreement includes three parts: (1) Part I (“Agreement and Employer Information”) shall be completed by Employer (defined below); (2) Part II (“Employer Execution”) should be signed by a duly authorized representative of Employer; and (3) Part III (“Agreement Terms and Conditions”) state the obligations of ConnectYourCare (“CYC”) and Employer with regard the use and disclosure of health information that is protected by federal law as defined below (hereinafter “Protected Health Information”).
