The Client wish to purchase services issued by OCAS, therefore,the Client and OCAS acknowledge and agree as follows: Client will pay in cash or check in the amount disclosed above to activate a yearly service agreement. Client will pay $ on a month by month billing cycle beginning / / in accordance with the payment method selected by Client from the given price-list. A late fee of $25.00 will be charged if payment is not received on or before ten days of the scheduled due date.
