THIS CONTRACT FOR SERVICES (the “Agreement”) is hereby made, entered into this 24th day of February, 2012, to be effective as of the 1st day of July, 2011 (the “Effective Date”), by and between GUILFORD COUNTY, a body politic and corporate of the State of North Carolina, hereinafter referred to as “GUILFORD COUNTY,” and TRIAD ADULT AND PEDIATRIC MEDICINE, INC., a corporation with a place of business in Greensboro, North Carolina, hereinafter referred to as “TAPM,” and also known collectively as “the Parties.” This Agreement will remain in full force and effect for a period as set out below.
