This agreement is entered into by and between Azusa Unified School District ("District") and Azusa Pacific University through its Community Counseling Center ("APU") for APU to provide school-based counseling for District students or family members (K-12) at each of the District’s school sites, or at the APU Community Counseling Center office. In consideration of the mutual agreements set forth herein, the District and APU enter into this agreement ("Agreement") on the terms and conditions set forth below:
