This Class Action and PAGA Settlement Agreement (“Agreement”) is made by and between plaintiffs Neil Modino, Rita Ortiz, and Valerie Fowler (collectively, “Plaintiffs”) and defendant Laurel Avenue, LLC DBA Terracina Post Acute (“Laurel Avenue”). The Agreement refers to Plaintiffs and Laurel Avenue collectively as “Parties,” or individually as “Party.”
