These terms of service (these “Terms”) are a legal agreement between you and Mount Sinai Genomics, Inc. d/b/a Sema4 (“Sema4” or “we”, “us” or “our”), with offices at 333 Ludlow Street, Stamford, CT 06902, establishing terms and conditions under which you may access and use the services and features available in connection with Sema4’s products, Sema4’s Tests (as defined below), Sema4’s testing programs, and Sema4’s Platforms ("Sema4’s Platforms” include Sema4’s website located at sema4.com (the “Sema4 Website”) and any other website or application owned, operated, licensed, or controlled by Sema4 including the patient portal (“Patient Portal”), the health care provider and organization testing portal (“Testing Portal”), and any Sema4 application (the “Sema4 Apps”).
