The following terms and conditions govern this Doc2Go Agreement (“Membership Agreement”) between The MetroHealth System, a county hospital established and operated pursuant to Chapter 339 of the Ohio Revised Code, doing business as Doc2Go and the individual completing the Doc2Go electronic registration form (the “Patient”). This Membership Agreement commences on the 3rd business day following confirmation of your account registration by Doc2Go (for example, if Doc2Go confirms your account registration on Friday, your membership will become active to seek Covered Services, as defined herein, on the following Tuesday) (“Effective Date”).
