Ingalls & Snyder, LLC (“Ingalls” or the “Firm”), acting as an investment advisor registered with the U.S. Securities & Exchange Commission, recommends and introduces certain of its clients with managed accounts to Boston Private Bank & Trust Company (the “Bank”) for participation in the Bank’s Insured Cash Sweep program (“ICS Program”). Ingalls and the Bank are not affiliates. If, after reviewing this Client Agreement & Disclosure Statement as well the new accounts documents provided by the Bank, you are interested in participating in ICS, please indicate your agreement by signing and dating this document in the space provided.
