I, the parent/legal guardian, of the minor child requesting admittance to the SC State Football Youth Camp, voluntarily allow my child to participate in the above camp. I hereby accept and assume all risks, known and unknown, and assume all responsibility for any and all claims, losses, damages, injuries, disability, paralysis, or death, even if caused, in whole or in part of, by the negligence of SC State University with the exception of willful or gross negligence. In consideration of my child's participation in the camp, I waive and release all and any claims based upon negligence, active or passive with the exception of willful misconduct, that I may have in the future against SC State, it's officers, directors, employees, representatives, agents, or volunteers. I agree to hold SC State University harmless for any injury, including, but not limited to, injury, paralysis, permanent disability, wrongful death, which may occur to my child during this camp/activity.
