Scope of Service. Based upon the items checked and described below, the undersigned subscriber (“Customer”) agrees to purchase and Your Company (“YOUR COMPANY”) agrees to provide monitoring, repair, inspection and/or response services (the “Services”) with respect to the type(s) of system(s) checked below (here in after sometimes referred to as the “System”) at Customer’s facility, for the equipment described in the Schedule of Equipment.
