This agreement is made by The Optical Society (OSA) and the customer named below (“Customer”). Customer wishes to rent a mailing list consisting of names and addresses from OSA’s customer database (the “List”) in order to permit distribution of materials (the “Mailing”) to individuals on the List, and OSA wishes to rent the List to Customer. In consideration of the mutual promises set forth herein, the parties agree as follows with respect to the rental of the List.
