This Letter of Agreement (Agreement), effective upon the date of the last signature below (Agreement Effective Date), is entered into in order to specify the terms and conditions under which Santa Cruz-Monterey-Merced Managed Medical Care Commission, doing business as Central California Alliance for Health (the Alliance) agrees to provide funds through the Housing and Homelessness Incentive Payment Program to County of Merced, on behalf of the Human Services Agency acting as the Administrative Entity for the Merced County Continuum of Care (Partner).
