This Agreement is between the Harris County Medical Society (“HCMS”), 1515 Hermann Drive, Houston, Texas 77004 and (Physician Name) (“Physician”) or (Group Name) (“Physicians”), who is/are member(s) of the Harris County Medical Society, collectively “the Parties.” This Agreement is to memorialize the relationship between the Parties and the terms that govern the release of Protected Health Information to HCMS and the use and disclosure of Protected Health Information by HCMS consistent with HIPAA and the HITECH Act and the regulations promulgated thereunder.
