The Agreement of Understanding with Applicants for Services form is completed with an applicant for Division of Services for the Blind (DSB) Vocational Rehabilitation (VR) Services after reviewing the DSB VR Handbook. The form is signed by the applicant, parent or other representative if applicable, and the DSB VR Counselor verifying the individual’s understanding of the eligibility criteria, his/her intention to work, and his/her right to choose the vocational goal, services, service providers and methods of receiving the services.
