(hereinafter referred to as DENTIST) who is qualified and licensed to practice Dentistry in the Commonwealth of Pennsylvania, and Capital Advantage Insurance Company, a wholly owned subsidiary of Capital BlueCross (hereinafter referred to as PLAN). Whenever mentioned herein, the term DENTIST shall include all employees of DENTIST, all partners, dental associates, and all staff personnel under DENTIST’s direct supervision and/or control.
