THIS NETWORK PARTICIPATION AGREEMENT (the “Agreement”) is made and entered into as of , 2016 by and between CNY AIM, LLC (“Network”), a New York State limited liability company, and each physician or medical group executing a Network Participation Agreement on behalf of participating physicians and listed on Exhibit A hereto (each, a “Participating Member” and collectively, the “Participating Members”). Together, Network and Participating Members are the “Parties,” and each is a “Party.”
