This Tn5 Enzyme Material Transfer Agreement (“Agreement”) is between Illumina, Inc., a Delaware corporation having offices at 5200 Illumina Way, San Diego, California 92122 (“Illumina"), and , having a place of business at (“Recipient”), and is effective as of the date of last signature below ("Effective Date").
