THIS AMENDMENT NO. 3 modifies, revises, and amends the Contract entered into between the DIVISION OF MEDICAID in the OFFICE OF THE GOVERNOR, an administrative agency of the STATE OF MISSISSIPPI, hereinafter referred to as “DOM”, and EQHEALTH SOLUTIONS, LLC, a corporation qualified to do business in Mississippi, hereinafter referred to as “Contractor” and herein collective referenced as the “Parties”.
