Specialist responsibilities‌• Patients who are being transferred to adult services need to be reviewed by the specialist adult teams as per the trust internal governance process and to confirm that the current treatment is suitable and in line with the Bsol IMOC formulary• Assess the patient and provide diagnosis. Ensure the diagnosis is within scope of this shared care protocol (section 1) and communicated to primary care.• Use a shared decision making approach; discuss the benefits and risks of the treatment with the patient and/or their carer and provide the appropriate counselling, to enable the patient to reach an informed decision.
