This Pole Attachment Agreement is made and entered into pursuant to 47 U.S.C. 224(c), and Sections 54-3-1, 54-4-1, and 54-4-13 of the Utah Code on this day of , 20 , between Qwest CorporationQwest Corporation d/b/a CenturyLink QC, a corporation organized and existing under the laws of the State of Colorado and qualified to do business in the State of Utah, hereinafter “QwestCenturyLink,” and , a organized and existing under the laws of the State of , and qualified to do business in the State of Utah, hereinafter “ ,” (collectively, “the Parties”). The Parties mutually agree that the terms and conditions of this Pole Attachment Agreement, hereinafter “Agreement” and applicable law shall govern the Parties’ non-exclusive use of such poles owned by each PartyCenturyLink and located in the State of Utah as each may, upon application, permit the other to use. Both parties to the Agreement may be referred to as “Pole OwnerCenturyLink,” “Licensee,” “Party” or “Parties.”
