WHEREAS the Physicians desire to constitute a group arrangement as an unincorporated association (the “Association”) in order for purposes including but not limited to selecting representative physicians for the Association Governance Committee (PAGC); selecting representatives for the broader __________OHT Governing Committee (OHTGC); discussing clinical and policy matters regarding physician participation with respect to OHTs; and providing a forum for physicians to speak with one voice within the ___________OHT catchment area;
