This Agreement ( “Agreement”) is made effective this 30th day of June, 2024, by and between Lake County Health Department, with a principal place of business at 2900 West 93rd Ave., Crown Point, IN 46307 ("LCHD"), and St. John Fire Department, with a principal place of business at 11033 W 93rd Ave St John In 46373 ("Grantee"), collectively referred to herein as the "Parties."
