Adirondack Health (“AH”) and the Resident, next of kin, or Responsible Party agrees to the following terms and conditions relating to admission, retention and the provision of services to the Resident by AH. All services are provided in compliance with the New York State Code and Resident Bill of Rights. State and Federal Law prohibits discrimination based on race, creed, color, national origin, handicap, sex, age, marital status or sexual preference.
