The Clinic is a Direct Primary Care Practice (DPC) which delivers primary care services through its physician, Charles Sasser, MD (Physician), at 4720 Bluebonnet Blvd Ste B, Baton Rouge, LA 70809. In exchange for certain fees, the CLINIC agrees to provide YOU with the SERVICES described in this Agreement based on the terms and conditions contained in this Agreement.
