Contract Type FiledMay 14th, 2021Mountain Telephone Services will be provided to you (“you,” “your,” or “Customer”) on the terms and conditions set forth in this Service Agreement (the “Agreement”) by Mountain Telephone Rural Telephone Cooperative and its subsidiaries that own and/or operate the cable television, system, broadband, or phone services in your area (“Mountain Telephone,” “we,” “us,” or “our”) and in any applicable Tariff(s) on file with the FCC, state public utility commission(s) or other comparable state agency. Services may include, but are not limited to, Mountain Telephone cable television service (“Video”), Mountain Telephone High-Speed Internet service (“Broadband”) and Mountain Telephone voice services including those that may that operate over a broadband connection (“VOIP”) or other local facility (each a “Service” and collectively, “Services”).
