EXHIBIT B – ALA PARTICIPATION FORMAcademic License Agreement • August 5th, 2020
This Participation Form must be completed for each establishment that participates in the ALA. For any establishments that wish to be added subsequently, a Participation Form and an Annual Fee Worksheet (Exhibit A) must be completed and submitted for that establishment and then in the following Annual Period, that new establishment should also be included in the new Annual Fee Worksheet which is due 15 days prior to the end of each Annual Period of the ALA.
