THIS SUBAWARD AGREEMENT (this “Agreement”) is made by and between Samaritan North Lincoln Hospital, an Oregon nonprofit corporation (“Samaritan”), and Lincoln County Health and Human Services (“Subrecipient”). Samaritan has contracted with Subrecipient, a member of the Coast to the Cascades Community Wellness Network (CCCWN) to provide certain services (the “Services”) and deliverables (the “Deliverables”) identified in Attachment 1 (the “Statement of Work”) of this Agreement effective September 1, 2022. The parties to this Agreement, in consideration of the mutual benefits and promises of this Agreement, agree as follows:
