This Arbor Pharmaceuticals EZ-RX Program Pharmacy Services Agreement (this “Services Agreement”) dated as of , 2017 (the “Effective Date”) is made between Arbor Pharmaceuticals, LLC, a Delaware limited liability company, having its principal place of business at Six Concourse Parkway, Suite 1800, Atlanta, Georgia 30328 (“Arbor”) and
