understand treating my condition will require a plan/program involving medication therapy for pain management. There are numerous state and Federal laws and regulations regarding the use of prescription medications and specifically controlled substances such as opioids. The purpose of this agreement is to help this healthcare practice and I comply with these laws and regulations. I understand this agreement will provide me with information about my treatment plan, and the medications I am prescribed to alleviate my pain to help me understand my pain.
