The Department of State Health Services (“DSHS” or “System Agency”) and Tarrant County (“Grantee”), each a “Party” and collectively the “Parties,” to DSHS Contract No. HHS000284500001 (the “Contract”), now want to amend the Contract further. Under the terms and conditions of the Contract, Grantee will continue to provide DSHS with active surveillance and reporting activities for HIV/AIDS (“Services”).
