The Housing Authority of Mayfield, herein called “PHA” or ”Authority”, and the person(s) named below, hereinafter called “Resident”, have executed this lease Agreement this 22 day of_02 20 21. The term of this lease shall be for one year and shall renew automatically for one year, unless terminated pursuant to this lease, by operation of law, or the Authority’s Admissions and continued Occupancy and Procedures (ACOPP). The parties agree and acknowledge that the ACOPP, Handbook, Board adopted Policies, and HUD regulations thereto adopted from time to time are incorporated herein and made a part by reference as if fully repeated in this Lease. A copy of the ACOPP, Handbook, and Board adopted Policies are posted at the Authority’s Office at 312 Brookside Dr., Mayfield, Kentucky 42066. A copy of these documents can be furnished upon request at the expense of the person making the request as provided by this lease.
