This Agreement is effective as of the date set forth on the attached Purchase Schedule (the “Effective Date”), by and between MedBridge, Inc., a Washington corporation (“MedBridge”), having a principal place of business at 1938 Fairview Avenue East, Suite 101, Seattle, WA 98102, and the entity set forth on the attached Purchase Schedule, having a principal place of business as set forth on the attached Purchase Schedule (“Customer,” and together with MedBridge, the “Parties,” and singularly, the “Party”).
