The person, partnership, or corporation whose name is set forth above (“Buyer”) warrants that the foregoing information is true and correct and acknowledges that Buyer is supplying such information to induce Lafayette Textile Industries, LLC. (“Lafayette Textile Industries, LLC.”) to accept Buyer’s checks for purchases of merchandise from and to extend credit to Buyer.
