This Referral Partner Agreement (this "Agreement"), dated as of , 20 (the "Effective Date"), is entered into by and between FOOTBRIDGE FOR FAMILIES, INC., with a principal address at 541 Glen Arden Drive, Pittsburgh, PA 15208 ("Footbridge"), and , with a principal address at (the "Referral Partner"). Footbridge and Referral Partner are referred to herein individually as a "Party" or collectively as "Parties".
