Thank you for your interest in participating as a Supervisor in American Denturist School’s (“ADS”) externship program (the “Externship Program”). Enclosed please find ADS’s Supervisor Application, Agreement and Release. Please read the Agreement carefully. If you agree to its terms, please complete the Agreement and return it to ADS. You must submit the completed Agreement to ADS prior to beginning direct supervision and training of any extern. Please let us know if you have any questions regarding the Agreement.
