By signing this agreement you give permission Dr. Rochelle Calvert, licensed clinical psychologist to provide psychotherapy for you. Please read this information carefully, and note any questions so they can be discussed. Please also note that in this document the words “client” and “patient” are used interchangeably, and refers to all persons participating in the therapy with me as part of this treatment agreement.
