We, the parents/guardians of (the “Player”), have read the information set forth herein, in the Club and USAV handbooks and presented at the parent meeting concerning the policies and practices of Tampa Bay Volleyball Academy, LLC, a Florida limited liability company d/b/a OTVA (hereafter called the “Club”) and agree to abide by the policies and Code of Conduct established by the Club. We agree that, having been selected to a team, we will allow the Player to join the Club for the 2017-2018 Club season. We understand that we are responsible for all fees due by the Player as well as the Player’s transportation to and from all practices and tournaments. In the event the Player quits or decides to discontinue playing, we understand that we remain responsible for paying the entire amount due for the season. We have read the Club and USAV Parent Handbooks, have attended the parent Meeting and understand the time and financial commitment involved in practice, competition and travel. We have
