THIS MASSLIGHT BUSINESS ASSOCIATE ADDENDUM (this “Addendum”) is entered into as of DATE, 2023 (the “Addendum Effective Date”) between CUSTOMER_NAME (“CUSTOMER”) and MassLight, Inc.
