This Agreement is entered into as of between Rush University Medical Center (“Rush”) and (“School”). As part of the Graduate Medical Education Program of School, Residents have the opportunity of receiving part of their required and/or elective training outside School at an affiliated location. School and Rush do hereby agree to cooperate in the development and training of School’s Residents while at Rush.
