THIS AFFILIATION AGREEMENT (the "Agreement") is made as of this first day of March, 2008 by and between (School Board of Clay County), hereinafter referred to as "School" and Orange Park Medical Center, authorized to do business in the State of Florida at 2001 Kingsley Ave, Orange Park, Florida 32073, hereinafter referred to as "Hospital".
