Child Admission Agreement & Health Assessment for each child Student Schedule Signed Parent Agreement Signed Photographic Authorization Form Signed Field Trip Permission Form If applicable, state assistance received, or the 1st month of tuition payment received $100 Registration fee paid at the time of enrollment (additional children $50 registration fee) The office must receive complete paperwork 48 hours (minimum) before a student can attend their first day.
