it may be amended, modified, or supplemented from time to time (collectively, this “Agreement”), is effective as of the effective date listed on the applicable order form (“Effective Date”), by and between the OrthoFX, LLC, or its designated subsidiary or affiliate who has signed the order form (“Administrator”), on the one hand, and the “Practice” and “Owner” identified on an order form, on the other hand (each, a “Party” and together, the “Parties”), and supplements and governs any such order form that is entered into by the Parties and referencing these terms and conditions. In the event of a conflict between any order form and these terms and conditions, the order form shall govern and control, only to the extent of the conflict.
