SERVICES AGREEMENT (“Agreement”) is entered into this day of , 20_ (the “Effective Date”) by and between AXIS WORLDWIDE SUPPLY CHAIN & LOGISTICS, INC., a Missouri corporation with its principal place of business at 100 Chesterfield Business Parkway, Suite 120, Chesterfield, MO 63005 (“Axis”) and the customer listed below (“Customer”). Axis and Customer are each sometimes referred to herein as a “Party” and together as “Parties”.
